MORPH Deflectable Catheter Enables Lumbar Artery Coil Embolization to Treat Type II Endoleak

Background
An 84 year old male patient with a 5.8 cm abdominal aortic aneurysm (AAA) underwent Endovascular stent graft implantation (Zenith by Cook, Inc.). Patient had past history of coronary artery disease with stable angina. At 6 months, CT angiography showed the AAA had grown to 6.2cm in diameter with a type II endoleak.


Procedure
Access was gained with an 0.035” guide wire and an 8F Diagnositc sheath. A BioCardia MORPH Universal 64 cm Deflectable Guide Catheter was used to canulate the internal iliac artery for implantation of coils (Cook, Inc.) into the AAA and lumbar artery to close the type II endoleak. The treatment of lumbar artery leak is shown in Fig 2.


Outcome
Use of the MORPH Deflectable Guide Catheter resulted in not only easy canulation of the lumbar artery orifice, but also provided superb guide support which allowed for advancement of the guidewire and catheter through a lengthy distance of tortuous vessel, followed by successful coil implantation. The catheter is simple and effective as a tool for accessing the internal iliac artery.
